Information we collect

Through this website we may collect the information you voluntarily provide, such as your name, email address, phone number, and the content of any message you send us. Our scheduling partner may collect additional information required to book and confirm an appointment.

We may also collect limited, non-identifying technical information such as browser type and pages visited, used only to keep the site working properly.

How we use your information

Information submitted through this website is used to respond to your inquiry, schedule and coordinate care, and carry out normal treatment, payment, and healthcare operations. We do not sell your information, and we do not share it for marketing purposes.

Protected health information (HIPAA)

As a healthcare practice, Restore & Thrive follows applicable federal and Maryland privacy and confidentiality requirements for protected health information, including HIPAA. Your clinical records are maintained confidentially and are disclosed only with your written authorization or as otherwise permitted or required by law — for example, in cases involving imminent risk of harm, suspected abuse or neglect, or a valid court order.

A full Notice of Privacy Practices is provided to patients at the start of care and is available upon request.

Website communications are not secure

Email and website forms are not fully secure methods of communication. Please do not include detailed medical information, diagnoses, medication details, or urgent clinical concerns in a website message. Clinical matters should be discussed during an appointment or through the practice's secure patient portal.

Your rights

You may request access to your medical records, request an amendment, request an accounting of certain disclosures, or ask us to restrict how your information is used. To make a request, contact the practice directly.
